Registered Practical Nurse – Out of the Cold Program

JOB DESCRIPTION – (Full-Time Contract) Registered Practical Nurse
Duration – (4 months) November – March 2021
Shift Focus – Monday – Friday; (5pm to 1am) *1 opening

St. Leonard’s Place Peel (SLPP) is a non-profit charitable organization that provides transitional and reintegration housing for men dealing with situations of homelessness, mental illness, addiction and/or conflict with the law.

SLPP is opening an Out of the Cold program from November-March and looking to recruit staff for the 4 month contracts. We are looking to recruit people who are committed to client care, have experience working with men that have experienced homelessness and have excellent communication skills.

Key Responsibilities

1. Welcome clients to the program
2. Complete intake paperwork and create/maintain electronic records
3. Complete regular checks of the sleeping area to ensure client and staff safety
4. Stay on site during the entire shift
5. Communicate with shift partner regularly throughout the night
6. Assist with referrals by liaising externally with referring agencies
7. Nursing duties such as, performing vitals, dressing changes, conducting follow-up health assessments as per scope and reporting changes in guest’s condition
8. Provide client support services through Health education; planning, organizing and providing health education on men’s health issues and healthy living
9. Regularly assessing guests and documenting changes in their records as per CNO standards
10. Tracking data/statistics to capture healthcare related outcomes and maintaining documentation as per policies
11. Conduct daily sanitization of facilities to ensure the health and safety of guests
12. Responsible for tracking of health concerns reported
13. Provide additional support to guests when required

14. Screening all guests for COVID as per Public Health guidelines and temperature checks will be performed as required
15. Other duties as required

Required Qualifications and Skills

• Diploma in Practical Nursing from an accredited institution
• Current registration with the College of Nurses of Ontario in good standing
• Demonstrated experience working with men that are homeless and may have substance use and mental health issues
• Exceptional communication skills
• Collaborative and knowledgeable of community resources
• Strong listening skills
• Superior problem-solving skills
• Excellent time management and organizational skills
• Proficient computer skills – MS office programs
• Current CPR/First Aid required
• Crisis Intervention, GPA, de-escalation training an asset
• Clear vulnerable sector check
