Title: Registered Practical Nurse

Reports to: Director, Health Services/ Nurse Practitioner

Reporting to the Director, Health Services/ Nurse Practitioner, the Registered Practice Nurse (RPN) will enhance the effective management and quality of nursing practice in keeping with the Standards of the College of Nurses of Ontario and the policies of St. Leonard’s Place Peel (SLPP).

Key Responsibilities:

• Working in conjunction with the Nurse Practitioner (NP) to oversee the medication management process at SLPP
o Developing and delivering training, protocols, and processes for providing medication access
o Overseeing medication access, participating in medication incident investigations, liaising with pharmacy

• Clinic support
o Screening, triaging and prioritizing client’s medical concerns, co-ordinating NP client referrals and booking appointments
• Nursing duties
o Performing vitals, dressing changes, conducting follow-up health assessments as per scope and reporting changes in client’s condition
o Administering monthly psychotropic medications and performing metabolic monitoring as per protocol
o Working with the NP and health team to develop individualized care plans for client’s with complex medical needs and severe mental illness.

• Client support services
o Ensuring that residents are supported to attend medical, hospital and counselling appointments, attend SLPP programing and take medications as prescribed
o Health education; planning, organizing and providing health education on men’s health issues and healthy living; liaising with off-site agencies to deliver specified on-site health education classes

• Documentation
o Regularly assessing clients and documenting changes in client’s electronic record as per CNO standards
o Tracking data/statistics to capture healthcare related outcomes within the health department
• Policy development
o In collaboration with the NP, assist in identifying and implementing required policies, procedural or practice change which improves the health care services at SLPP
• Other duties as required

Required Qualifications and Skills:

• Diploma in practical nursing from an accredited institution
• Current registration with the College of Nurses of Ontario in good standing
• Minimum of 2 years’ experience working with high risk adults with persistent mental illness
• Demonstrated experience dealing with the homeless, individuals with severe mental illness (SMI) and/or addictions
• Deep sense of human compassion and dedication to the reintegration goals of the offender population
• Demonstrated ability to understand the dynamics of offender behaviour and assess their needs and risks
• Skills in communicating effectively, both orally and in writing
• Able to understand diversity and cultural differences while demonstrating culturally competence
• Familiarity with the dynamics of offender behaviours and sensitivity to their needs
• Strong listening and problem solving skills
• Time management and organizational skills
• Proficient computer skills – MS office programs
• Current CPR/first aid required
• Crisis Intervention, GPA, de-escalation training an asset

Physical Requirements

• Some data entry, with extended periods of time sitting stationary in front of a monitor while keyboarding
• Ability to balance all job requirements in a busy atmosphere, including responding promptly to all communication
• Occasional standing for extended periods of time and some occasional lifting may be required
