St. Leonard’s Place Peel provides community based transitional housing for men in conflict with the law or who have become homeless due to situations of mental health and/or addiction.
SLPP is currently seeking a social worker (MSW designation) with a specialized background in mental health and addictions. This position, reporting to the Director, Operations, will coordinate care plans for individuals with mental illness at risk of homelessness. The successful candidate will have working knowledge of serious and chronic mental illness, related treatment modalities, interventions and techniques. Experience carrying a caseload, leading assessments, developing and implementing psycho-education groups as well as developing treatment plans and using local community resources is required. Additional responsibilities include:

Key Responsibilities
Direct Service
• Lead client screening and assessments at the intake level
• Using appropriate assessment tools to observe, record and report on an individual’s functioning
• Providing clinical assessment and clinical counseling to clients
• Developing appropriate care plans with clients
• Co-develop and implement new evidence-based psycho-education, clinical and therapeutic programs that support the needs of SLPP’s clients.
• Provide on-going clinical case management functions in accordance with best practice standards
• Ability to work within and assist clients in navigating through complex mental health systems
• Foster and maintain communication with the consulting psychiatrist and medical team for input regarding the intersection of clients care plan with medication compliance, clinical assessment and counseling
• Develop and implement client education / capacity building groups that respond to the developmental needs of clients
• Provide input, advice and feedback on programs implemented
• Work with department supervisor to identify and improve processes that will enhance program delivery

• Provide accurate reporting on quantitative and qualitative program outcomes and impact
• Maintain electronic records, reports and statistics necessary for program functioning and evaluation
• Handle inquiries, in person or on the telephone, from service providers, prospective residents and family members

• Masters of Social Work degree and eligibility for registration with the Ontario College of Social Workers and Social Service Workers
• Three to five years’ experience working in mental health
• Working knowledge of Justice issues
• Experience with community development practice, crisis/trauma counseling, working knowledge of Mental Health Act, Substitute Decision Act and Health Care Consent Act
• Experience managing response to crisis situations and experience working as part of case management support team
• Demonstrated ability in meeting-facilitation, oral presentations and situations of conflict
• Demonstrated ability to relate to SLPP’s client population
• Advanced understanding of treatment programs, mental health terminology, including diagnostic categories; knowledge of privacy guidelines (PHIPA/FIPPA) and ability to communicate difficult/sensitive information
• Ability to work in diverse community with clients of various ages and cultural backgrounds
• Demonstrated experience utilizing clinical interventions with adult and youth males that include motivational interviewing, solution focussed therapy and cognitive behaviour therapy.

• Knowledgeable about diversity, equity and inclusion and ability to identify potential barriers
• Demonstrated experience facilitating clinical programs for adult males
• Experience in database management software
• Proficiency in the use of Microsoft Office Suite software
• Excellent verbal and written communication skills
• Current CPR/First Aid certificate
• Clear Police check and Approved Corrections Canada Enhanced Reliability clearance.

Hours: Full-time (40 hours per week)
Salary Range: $55,972.43 – $62,085.82
Reports to: Director of Operations
